You can order blood tests by mail by calling 954. 929. 4814. As soon as you place your order, you will be sent a Request for Phlebotomy form, a bullet tube (if required), along with all of the instructions for preparation of the sample. If you need a blood drawing site near you, let us know so that we can order the drawing for you through Lab Corp in your area.(There is an extra charge for this service that you will have to pay to the drawing facility.) Shipping by overnight mail is a patient responsibility. All tests should be prepaid at the time you are ordering the procedures.

American Metabolic Labortaories are out of network of any and all insurance companies. However, for a nominal fee we can file an official reimbursement claim for you. A number of insurance companies require a referral from the individual's own M.D. or D.O. physician. (Medicare/Medicaid are not accepted.) In such a case, the doctor must indicate medical necessity i.e., ICD 9 diagnostic codes for each test ordered. The doctor must provide his/her UPIN. A legible copy of your insurance card and your date of birth must be enclosed. - Otherwise, our triple Board Certified, licensed pathologist will order your tests but will not be involved with your claim for you are not a patient of this doctor. The script you will receive for having your blood drawn is called, "Request for Phlebotomy" for Testing and Research. PLEASE, if any, indicate your diagnosis and date in some detail.

You should take the Request for Phlebotomy form, provided, to any of the blood-drawing stations in your area. A phlebotomist will draw the appropriate specimens of your blood; then you will overnight ship your specimen(s) to the Laboratory. We will mail the test results to you or to your ordering physician . In the latter case, the ordering physician, by law, will receive the results and s/he needs to authorize their release to you. If our staff pathologist doctor ordered the tests, the authorization is automatic. These results will indicate if there are any abnormalities. In case of abnormal results , you should make sure to show them to your personal physician, who can determine if you have any serious problems and what you can do about them. RETURN POLICY FOR TEST KITS:

All returned kits must be unused, undamaged and include all supplies and paperwork. There is a $30 restocking charge. Shipping & handling fee will NOT be refunded. Returns are accepted and refund is credited back to you for up to maximum 30 days after ordering the tests.
